I do hereby agree that I am aware of the potential risks associated with getting a tattoo and that a tattoo is a permanent change to my appearance.

I affirm that I have been given the opportunity to ask any questions regarding the application of my tattoo, and that the independent artists operating within Bombshell Tattoo Galerie have answered to my total satisfaction.

I affirm that I am entirely responsible for checking the spelling of any and all wording, as well as the correctness of any dates in my tattoo.

I agree that any touch-up work required due to my own negligence or failure to adhere to the aftercare instructions listed on this website, will be performed at my own expense.

I affirm that I am not under the influence of alcohol or drugs of any kind, and am voluntarily receiving a tattoo of my own free will.

I affirm that I do not have any current infectious disease, including but not limited to: Covid19, influenza, Hepatitis, tuberculosis or unmedicated HIV (Aids)

I affirm that I am not currently experiencing Haemophilia, or taking blood thinners, antibiotics or acutaine.

I affirm that if I have diabetes or epilepsy I shall notify the independent artists operating within Bombshell Tattoo and am responsible for any complications arising from my own physical condition.

I release all photographs taken of myself and my tattoo within Bombshell Tattoo Galerie as well as any photographs where my tattoo is the main focus of the image taken thereafter to both the original artist and Bombshell Tattoo Galerie for promotional use only.

I affirm that I will follow the provided instructions on the care of my healing tattoo, and I acknowledge that it is possible for a tattoo like any other wound to become infected and will exercise due diligence in following the provided care instructions.

I affirm that I have been provided with ample opportunity to consult upon my artwork design and am able to modify my design to my liking should I require to do so until I am happy with my artwork.

I acknowledge that I have been given adequate opportunity to read this document and I understand that by signing this document I am waiving any rights to pursue legal action against the artist or Bombshell Tattoo Galerie.
